Doctor and Patient Relationship
Occasionally a total breakdown of the relationship between doctor and patient occurs and it is necessary for the doctor to request removal of the patient from the list. In this instance the doctor will discuss with the patient the reasons for removal and a letter will be sent to the patient explaining the reasons for the decision. The patient will be officially notified by Practitioner Services and informed of the date of removal from the list.
Any incidence of violent or abusive behaviour will not be tolerated and may result in immediate removal from Practice List.
Failing to keep appointments
Patients who fail to keep their appointments more than three times without notifying us may be removed from our list as they are denying appointments to other patients.
