If you live in our Practice catchment area and would like to register with the Practice please come to the surgery and fill in the necessary forms.  If you have your medical card please bring this with you.


It would be helpful to bring two forms of identification which confirms your address and date of birth.  If possible, one of which should contain a photograph e.g. passport.  


You can still register with the practice without identification but it does help the correct matching of an existing NHS record and enabling the smooth transfer of your medical record from your previous practice.

Patients are registered with the Practice rather than an individual doctor but retain the right to see the doctor of their choice.  Please inform the Practice receptionist if you prefer to see a particular doctor.

The reception team is here to assist if you have any questions.

Your application form and health questionnaire will be checked by one of our GPs or Nurse.  You may be invited in to discuss any ongoing health conditions or medications.

 


Your registration cannot take effect until you have signed the necessary forms. The form is for patients who expect to be resident in the area for more than 3 months (if not, please contact the surgery directly instead of using this form).    



Note that we cannot guarantee that we shall accept you as a patient since, for example, it may be that your address is not in our catchment area.
