Subject Access Request (Access to Health Records)
You have the right to access the contents of your medical records. Requests should be made in writing to the General Manager. We have also created a form which is available to download (below).
Copies of your records will be supplied within 1 month of receiving a valid request. In some exceptional circumstances this may take longer but if this is the case you will be advised prior to expiry of the initial 1 month period.
